Teach Me Ms. Amahni Service Agreement

This agreement outlines the terms of your student's journey here to ensure everyone is on the same page from day one. Every checkbox is required before this form can be submitted.

Payment Authorization & Autopay Agreement

A valid payment method on file is required to enroll and remain active. This form authorizes recurring charges according to your program's billing terms. Please read through each section carefully before signing.

Audio & Video Recording Acknowledgment

Security cameras are in use in all session spaces for the safety and accountability of every student and family. This form explains the recording policy. Each section must be read and acknowledged before submission.

AI-Assisted Tools Acknowledgment

AI-assisted tools are woven into how sessions are supported, from preparation to documentation. These tools are a standard condition of enrollment. Each section must be read and acknowledged before submission.

Media & Marketing Consent

Every item on this form is independent and optional. Check what you are comfortable with and skip what you are not. If anything changes down the road, just reach out and your preferences will be updated right away.
