These are the last few forms before we are fully on our way. Each one opens right here on this page. They are all pretty quick — just a few checkboxes and signatures. All five need to be completed by the end of your student's first Academic Placement and Orientation session.

The first four forms are required. The last one is completely optional, and every item on it is independent.

AI-assisted tools are woven into how sessions are supported, from preparation to documentation. These tools are a standard condition of enrollment. Each section must be read and acknowledged before submission.

Every item on this form is independent and optional. Check what you are comfortable with and skip what you are not. If anything changes down the road, just reach out and your preferences will be updated right away.
